HTML - Can't Find Index.htm

Problem

I've published my book but when I try to preview the HTML pages, it says it can't find the index.htm.

Reason

Your Book is missing a Table of Contents object which it needs for the HTML output to publish correctly.

Solution

Add a Table of Contents object to your Book and publish again.
